The dental insurance data for 1997 come from the following 20 self-selected states and is thus not nationally representative: Alabama, California, Colorado, Florida, Idaho, Indiana, Maryland, Mississippi, Missouri, Montana, Nevada, New Jersey, New Mexico, New York, Ohio, Tennessee, Texas, Utah, Virginia, and West Virginia. The 2001 dental insurance data come from the following 14 self-selected states: Arizona, Idaho, Iowa, Missouri, Nebraska, New Hampshire, North Carolina, North Dakota, Pennsylvania, Rhode Island, South Carolina, Virginia, Washington, and Wisconsin.
